Thank you for contacting me to schedule an appointment for acupuncture in Binghamton!

Please print and fill out the three intake forms below, and bring them in with you when we meet.

My office is located on the second floor of the NY Skin and Vein Center at 75 Pennsylvania Ave. When you come in the main door, there is a stairwell on the left that you’ll go up and there’s a waiting area upstairs for Healthy Living Acupuncture.

The first session is $108 and then it goes to $88 after that, cash or check only. All sessions are one hour.

Please wear loose-fitting clothing that can be pulled up over your elbows and knees.

Please read the office safety protocols listed below that we’re following during this time of COVID-19.

All patients must wear a mask to appointments.

COVID-19 Office Safety Protocols

During this time of COVID-19, there are some office safety protocols I am implementing. I realize some of these may be inconvenient, but for everyone’s health and wellbeing, I’m following the Centers for Disease Control (CDC) and Occupational Safety and Health Administration (OSHA) recommended guidelines to the best of my ability.

The main points are:

I’ll be in touch a day or so before your appointment (preferably by text) to make sure you don’t have any COVID-19 symptoms. When you arrive, I’ll text you when it’s time to come in. Please wear a mask and wash your hands upon arrival.

Below are more expanded details to read through so you’re familiar with everything I’ll be doing to keep you and all my patients healthy:

 

BEFORE YOUR APPOINTMENT

 

  1. Prescreening: I will contact you (preferably by text) about 24 hours before your appointment to confirm that you don’t currently have any symptomsrelated to COVID-19 infection (fever of 99.6 or higher, cough, body aches, shortness of breath, and fatigue are the most common symptoms; additionally chills, headache, runny nose, loss of taste or smell, muscle pain, sore throat, diarrhea, and nausea), and that you haven’t been around anyone you suspect may be infected with COVID-19.
  1. Waiver: there is aone-time consent form specifically regarding COVID-19 to sign. It’s attached below. Please print it ahead of time and bring it in with you to your appointment. Otherwise, I will have one available for you to sign upon arrival.
  1. Pleasehave payment ready ahead of time (cash or check) to limit surfaces touched during your visit.

UPON ARRIVAL TO THE OFFICE

  1. Please wait in your car until you receive a textfrom me letting you know it’s time to come in for your appointment. If you don’t text, please just come in exactly on time. I will be prompt with all appointments.
  1. It’s necessary for everyone to wear a maskupon entering the office and during your session.
  1. Please sanitize your hands before entry.
  1. Upon entry, I’ll take your temperatureusing a no-touch thermometer to check that you don’t have a fever. If you do, unfortunately we will need to reschedule your appointment for another date, and I will recommend you see your medical doctor for evaluation.
  1. Before coming into the treatment room, please wash your hands very well with soap and waterin the bathroom.
  1. It’s recommended that companions remain outside in your car. If you require assistance coming in and out of the office, your companion will also need to follow steps 2-5 and stay in the room with you (the waiting area is closed).

In addition, I will be wearing a face mask, gown, and protective glasses; using disposable paper on the tables; and sanitizing all major surfaces using an EPA approved COVID-19 disinfectant in the treatment rooms between every patient.
